Please find below our parental consent form for the administration of medicines. Please can parents print off the form, complete and bring into school along with your child's prescribed medicine, there are also paper copies of the form available from the school office. Unfortunately we are not able to administer any medication that has not been prescribed by a Doctor, however we are happy for parents to come into school and administer this themselves.
